I understand sound baths are offered to enhance relaxation, and overall health and well-being. However, all participants have individual experiences unique to them.*

I understand that there may be medical conditions that could be exacerbated by the sound bath experience. This includes, but is not limited to conditions such as severe mental health conditions, epilepsy, vertigo, high blood pressure, conditions with pacemakers and metal implants, certain heart conditions, recent surgeries, or traumatic brain injuries. I understand participants should seek the medical advice from licensed medical providers prior to the sound bath participation.*

I understand that Blossom Studio and Mila Boyd, as Sound Practitioners, are not not licensed health professionals. As Sound Practitioners, they will not provide medical advice, diagnose, prescribe, or perform medical treatment. *

I affirm that I alone am responsible to decide whether to participate and participation is at my own risk. I hereby agree to irrevocably release Blossom Studio LLC, its practitioners, agents, and affiliates and waive any claims of liability against them. *

I have read and fully understand and agree to the above terms. I am signing this agreement voluntarily and recognize that my signature serves as a complete and unconditional release of liability to the greatest extent allowed by law.
